Adding a Fallen Firefighter’s Name to the List of the Fallen

The list maintained by Canadian Fallen Firefighters Foundation is for those firefighters who have died in the line of duty. There are two pages on this web site that will give you the information you need to have the name of a fallen firefighter added to the list.

Read the Line of Duty Death Definition. This explains the reasons for which a firefighter will be added to the memorial list.

Download Firefighter Line of Duty Death Application and print out the application form. The form is in PDF format. You will need Acrobat Reader to access it. (Click here for French version of the application)

The application form needs to be filled in completely with supporting documents attached to show that the firefighter named on it died in the line of duty. There is a mailing address on the form that tells where to send the form and documents.

In order to attend the annual ceremony applications must be received by March 31. (Please note the important change in the application deadline date.)
